Japan’s Universal Artificial Blood Could Save Millions. The Price Is Still the Question.

In a lab at Nara Medical University in Japan—not the National Defense Medical College, as some reports have wrongly said—a medical shift is taking shape: universal artificial blood that works with every blood type, carries oxygen, and helps control bleeding without a drop of fresh human blood. Since March 2025 the work, led by Professor Hiromi Sakai, has been in human trials. Sixteen volunteers have shown only mild effects such as fever or a rash. Shelf life of up to two years at room temperature, no fridge required. The pitch is ambulances, battlefields, and earthquake zones that no longer wait on a blood bank. Japan wants practical use before 2030. Cost, scale, and what happens to donation systems are still open. A fix for the global shortage—or a breakthrough with a shadow? Here is why it matters, what it could change, and what stands in the way.
Why universal artificial blood matters
The product, hemoglobin vesicles (HbV), is a synthetic stand-in for red cells, made from hemoglobin pulled out of expired donated blood and wrapped in a lipid membrane. The purple fluid has no blood-type antigens, so it is compatible with anyone. Newsweek notes the usual transfusion problems it is built to dodge: infection, mismatch, immune reactions, and a short shelf life.
- Universal match: No ABO or Rh markers, so it can go into any patient without typing. In trauma, seconds count.
- Stability: Up to 2 years at room temperature and 5 years refrigerated, versus 42 days for donated blood, according to Kyodo News. Ambulances, rural clinics, disaster caches.
- Early safety: In 2022 and 2025 trials, 16 volunteers received 100 to 400 ml of HbV with no serious events—fever or mild rash only, according to Medscape.
- Global gap: WHO estimates 117.4 billion donated units are collected each year, and it is still not enough. In low-income countries only 40% of need is met, Nara Medical University says.
Professor Sakai: “There is no safe substitute for red blood cells. This technology could save lives where donated blood is not available.” On X, @UHN_Plus and others called it a change in how disaster care could work.
A tool for emergencies—and more
Artificial blood is aimed at the worst hours in emergency medicine:
- Natural disasters: Earthquake-prone Japan could treat victims without waiting on blood banks—the logistics failure of the 2011 Tohoku quake, in a bag.
- Rural and military use: Remote ambulances and field hospitals could carry HbV with no cold chain and no crossmatch.
- Fewer donors: An aging country that has lost 20% of its donors since 2000 could ease pressure on collection systems.
- Other uses: Professor Teruyuki Komatsu at Chuo University is looking at HbV to stabilize blood pressure in hemorrhage and to treat stroke, according to BusinessToday.
Goldman Sachs puts the blood-substitute market above $10 billion by 2035, with Japan ahead of efforts such as ErythroMer in the U.S., which DARPA has funded with $46 million.

The risks: is this the final answer?
The “universal solution” story still has holes:
- Still tied to donated hemoglobin: Expired blood is the feedstock. A Reddit comment put it cleanly: “The real leap is synthesizing hemoglobin in the lab.” Expired units are not an infinite supply, which caps scale.
- Production cost: HbV is expensive. A unit of lab-grown blood ran $90,000 in 2013 and is now about $5,000—still far above the $215 for a donated unit in the U.S., according to Al Jazeera.
- Efficacy is unproven in the field: Current trials test safety, not performance in massive trauma. In 2022, HbV carried oxygen; it has not been tried in severe human hemorrhage, Medscape notes.
- Long-term side effects: Mild effects so far. Large doses (over 400 ml) or long use could turn toxic, as earlier substitutes such as Hemopure did, according to Science.
- Donation systems: If HbV crowds out donated blood, plasma and platelet supply could shrink—the fear an expert voiced in The Japan Times.
- Regulators and who can pay: FDA or India’s CDSCO approval will take years. High cost could lock it in rich countries, the same access problem as Apple’s Vision Pro.

A correction on where this work lives
Some coverage names the National Defense Medical College in Tokyo. The main project is at Nara Medical University, led by Hiromi Sakai. A separate 2019 study at the Defense college, reported by IFLScience, built a substitute with synthetic platelets and red cells, tested in rabbits with 60% survival—and it is not in human trials. Early press likely blended the two programs.
The future of artificial blood
Japan is out in front of a technology that could rewrite trauma care. If the 2025 trials show it works, Nara Medical University wants production scaled by 2030, making Japan first to use artificial blood at volume. The path is not clean. Like Silicon Valley automation—efficiency on the slide, jobs on the chopping block—or Google’s HeAR and false positives, artificial blood has to clear access and safety, not just a demo.
The “blood without blood” line is a good one. It skips the hard questions: who funds production, how it gets distributed fairly, what happens to donors. A Reddit user: “A big step, but it does not erase the need for real blood.” This is not science fiction. It is a concrete chance to save lives. It only counts if it does not leave people behind.
